Contract for Postpartum Doula Services

This agreement is between __________________________________ (client) and Kate Hogeland for the purpose of providing postpartum doula care.

I agree to provide nonmedical physical, emotional and informational support after the birth of your baby.
I will help with self-care recovery, postpartum comfort measures, infant care (including bathing, feeding, safety issues, and swaddling), parenting information, and provide assistance with learning to feed and take care of your baby. I am a Certified Breastfeeding Educator and can provide breastfeeding support. If something is beyond my scope, I can refer you to a lactation consultant.  Practical support will include preparing simple meals and snacks, getting frequent drinks, and answering the phone and door if desired. The tasks I provide each day will depend on the priorities discussed by all parties before beginning services and on your particular needs that day.

I am happy to do some basic food preparation or light housework. Food preparation may consist of sandwiches, oatmeal, eggs, etc.  Cleaning may consist of laundry for mom and baby, sweeping, picking up toys, or dusting in areas. Kitchen cleanup may include emptying the dishewasher and wiping down counter tops. I do not do major housecleaning tasks such as windows, cleaning of bathrooms, or yard work.

As a Postpartum Doula, I will NOT transport you or any member of your family.  However, I can ride with you if you need me to go somewhere with you.  As a postpartum Doula I also do NOT take over all care of the baby.  My goal is to help you become confident and secure in your own abilities as a new mom and family.

I do Not diagnose any medical conditions for mother or baby, but will refer you to an appropriate health care provider if I notice anything of concern.

My fees for Postpartum Doula services are   dollars per hour.  . If you request that I run errands for you on my way to your home, the hour begins upon arrival at the first stop.  I require a minimum of 15 hours to be completed within two weeks.  Of course, if you would like to purchase more than 15 hours we can arrange as many as you need.

A one hundred dollar deposit is required when you sign this contract.  If you need to cancel 50% of the deposit will be returned.  You will be billed weekly for my services. Your invoice will include a summary of hours for that week. Payment is due upon receipt of invoice.

.In the rare event that I need to cancel a visit, I will call and either reschedule or, if you would prefer, find a replacement for the day.

 

 

Services are billed on an hourly basis with a minimum of 3 hours per visit and minimum 15 hours total..
